​Packages

Packages are a one-time purchase and include a set number of treatments at a discounted rate. All packages are valid for 6 months from the date of purchase. No refunds or exchanges on packages.

 

Cancellation Policy

  We kindly ask for at least 24 hour's notice when canceling or rescheduling an appointment. Cancellations made with less than 24 hour's notice will be charged 50% of the scheduled service, and no-shows will be charged 100% of the scheduled service. Patients with a package, cancellation and no-show fees will be charged separately and will not be deducted from or applied toward the package. In cases of repeated or habitual late cancellations, we reserve the right to charge 100% of the scheduled service for cancellations made with less than 24 hour's notice.

 

​We reserve the right to discontinue the patient-provider relationship at any time when appropriate.​

 

One Year Policy

If you have not been in for treatment for one year or more, another patient evaluation is required.

 

We do NOT do any insurance billing (EXCEPT for the Natera Cancer Testing)
